I'm tired of "waiting to die"

I've fought with obesity since childhood. When I graduated high school I was 220 lbs, and my weight steadily climbed year by year. By the time I was 30, I weighed 275 lbs. God brought a great man into my life in May of 2004, and I married him in November of that same year. We immediately began trying to expand our little family, to no avail. The next ten years brought failure, grief, devastation, fertility drugs, depression, stress and two babies taken to Heaven. I have a hormonal disorder known as PCOS (polycystic ovarian syndrome) and several blood-clotting disorders (Lupus Anticoagulant, Anticardiolipin Syndrome and Compound Heterozygous MTHFR mutation). All of this contributes to my infertility and the blood clotting disorders led to my miscarriages. PCOS also is one cause of my weight problems.
In early 2015 I was finally officially diagnosed with hypothyroidism, after fighting for years to get a doctor to take me seriously.
I've had quite a few wake up calls recently, and I've come to the realization that I have to change the way I'm living (or rather, existing...I haven't been truly LIVING for a long time) or I won't see my 44th birthday. I feel like I'm not living my life, I'm simply existing. I feel like I'm just sitting here waiting to die. And I'm tired of that.
I am limited in what activity I can do, because of my size and some pretty bad back and knee injuries. I need a knee replacement, and have 3 completely degenerated discs in my lower back, both of which cause severe debilitating pain. The doctor won't do anything about the knee replacement until I lose 230 pounds.
I was also recently diagnosed as pre-diabetic, and I have fatty liver. My Dad died from liver cancer in August, 2015, and I am terrified that I will develop cancer as well.
I HAVE to lose weight. Not just to relieve my pain, but to save my life.
This is about my 100th try at losing weight, but I have to do it this time and get healthy or I know I'll be dead by 45. I refuse to let that happen.

My Goals:
Lose 50 lbs by Christmas.
Be more active.
Feel better about myself.
Complete a 5K.
Long term: Lose at least 200 pounds.
To not be in constant, chronic pain.
To lose enough weight to be able to get my knee replacement.
My Program:
Daily tracking with SP Android app and website.
Following a low glycemic index diet.
Measuring and weighing food.
At least 30 minutes of some type of activity daily.
Consistently drinking 10+ 8oz glasses of water daily.
Eating more fresh veggies and fruits, less processed and packaged foods.
